All health forms should be submitted through Magnus Health, which you can access directly or through the AFS Parent Portal,  a secure system for submitting required health information to AFS. If you have questions about required health forms, please contact Susan Harris, School Nurse.

In order to attend the first day of school, all students regardless of grade, must have a copy of their current immunization record on file in the nurses office.
